Where can I find an Explanation of Payment (EOP)?
Click on the “View Explanation of Payment” link. From there, you will be directed to a new site; at this site, enter in your login information. If you are not registered, please click on the Not Registered link to set up your account. Once you have signed in, simply click on the “Claim Search” link. You can find a claim by the member’s ID number or by the claim number. Once the claim is found, you can click on original EOP view to pull up this information.
How do I determine if a diagnosis or procedure code needs preauthorization?
We are unable to quote a benefit by a diagnosis or procedure code. Please provide a detailed description of the service for preauthorization to a member of our Customer Care Team at 1-800-869-7093 and they will let you know if the service requires preauthorization.
Where do I submit a preauthorization request?
Click on the “Submit a Preauthorization Request” link. From there, you will be directed to a new site; at this site, enter in your login information, if you are not registered, please click on the Not Registered link to set up your account information. Once you have signed in, simply click on the “Authorizations” on the top menu and you will be able to submit your preauthorization request.
Where do I determine if a diagnosis or procedure code is covered?
We are unable to quote a benefit by a diagnosis or procedure code. Please provide a detailed description of the service being provided and the code to a member of our Customer Care Team at 1-800-869-7093 and they will provide you with an accurate benefit quote.
Where should I submit claims?
We accept electronic claims through Availity using payer ID HMA01. You can also submit claims via mail to our claims address P.O. Box 85008 Bellevue WA 98015 or fax at 1-866-458-5488.
